Visits are available at a self-pay rate of $105.
• A Good Faith Estimate of charges will be provided before your visit.
• After your visit, a detailed superbill will be provided for submission to your insurance company. Reimbursement is not guaranteed and varies by plan, deductible, and out-of-network benefits.
• Medicaid and other government insurance patients: Cash-pay visits are not available for services covered by your plan.
